To initiate the person's functionality assessment process, an electronic application must be submitted through the e-disability.am system.
Medical documents confirming the health condition and the referral for the functionality assessment are retrieved from the "Armed" electronic healthcare system.

The application may be completed by the individual or their representative by entering the Public Services Number (PSN) or the reference number of the certificate confirming the non-issuance of a PSN.
If the application is completed by a representative, it is also required to provide a document confirming the representative's status—either a document certifying guardianship (trusteeship) or, for individuals over 16 years of age, a power of attorney—as well as the representative's Public Services Number (PSN) or the reference number of the certificate confirming the non-issuance of a PSN.

In the case of children under 16 years of age, the application shall be completed by one of the parents or the guardian (trustee) by entering the Public Services Numbers (PSNs) or the reference numbers of the certificates confirming the non-issuance of a PSN for both the person being assessed and the parent or guardian (trustee).
Foreign citizens, stateless persons, and individuals with refugee status must enter the number of the document certifying their right to reside in the Republic of Armenia.
