• Medical documents confirming the health condition and the referral for the functionality assessment are retrieved from the "Armed" electronic healthcare system;
  The application may be completed by the person or their representative through the "e-disability" electronic system by entering the public services number (PSN) or the number of the certificate on not having a PSN.

If the applicant holds the status of a representative, the following are also required:

• A document confirming the status of the representative: a document confirming the fact of being a guardian (curator), or a power of attorney in the case of persons above the age of 16;
• The representative's public services number (PSN) or the number of the certificate on not having a PSN.
• In the case of children under the age of 16, the application shall be completed by one of the parents or the guardian (curator), by entering:
• The public services number (PSN) of the person under assessment or the number of the certificate on not having a PSN,
• The public services number (PSN) of the parent or the guardian (curator).

Foreigners, stateless persons, and persons with refugee status must enter the number of the document confirming the right to reside in the Republic of Armenia.
